Join us on this date for an End-of-Life Planning Program: Advanced Care Planning, conducted by Madison Strong with St. Croix Hospice.
This program is an overview of advanced care planning, including advanced healthcare directives, and the role of family, loved ones and healthcare providers in decision making.
Participants will learn to evaluate personal values and wishes, potential healthcare concerns, and how treatment may impact quality of life.
